ROAD ISSUES LOGGING

This is an example of a fault logging form used to record road issues such as potholes or traffic light failures. The form automatically detects the users current location, if possible. Alternatively the user can enter the physical address and VisionForms will convert this to coordinates. Information collected on this form is pushed through automatically to Google Fusion Tables where it is displayed on an interactive map.
